To provide you with this care, it is important that we understand all of your needs and concerns. Attached is a general questionnaire that will provide us with pertinent information such that we can treat you with the whole person in mind. Please take a few minutes to fill out the questionnaire.

The Rockdale Rehab Physical Therapy strives to provide timely appointments for you. In order to achieve this, we must decrease out last minute cancellation and no show appointments. Please contact us if you are unable to attend a scheduled appointment. There may be a $30.00 administrative fee applied to your account id an untimely cancellation (12 hours or less) or a no show appointment occurs.

Also, please be aware that your insurance carrier will not cover Physical Therapy treatment when you also see your chiropractor that same day.   Please keep this in mind when scheduling your appointments.
